Dude, get it together. If you know you've made her unhappy... stop. Tell her why you've been nasty to her, and have her hold you to not being a jerk. If you have a good friend who is truly a friend, ask that person to keep you honest. Don't let this past year be a frustrating, next-to-waste of time for her. She's put that effort in because she thinks you're worth that effort - prove her right.
I'm actually in your girlfriend's position in this situation. I know you don't want to lose her because of how much you've changed for the better since you've been with her. But give yourself some credit. Maybe she gave you the courage or inspired you, but you are the one who has made so much progress. Try your hardest to trust her and not want to control the relationship so much. That's exactly what my boyfriend is doing and he's pushing me away. If you want to hold on to her, you need to try and relax. And TALK to her about it.
Be nicer to her! And tell her how you feel. I bet she will help you
